I, XXXX XXXXX S/o ShXXXX XXXX, aged about ___ years, R/o House No.N-XX/XX Ground Floor, SCG NoX/XX XXXXX XXXXX Delhi-110054, do hereby solemnly affirm and state on oath as follows:-

1.                  I am father / natural guardian of my daughter namely XXX, she is my first born child and her place of birth is ………

2.                 The true and correct date of birth my daughter XXX is ………. And applying for admission to Class ……… in School for Academic Sessions 2024-25.

3.                 That the deponent has no documentary proof regarding date of birth of his daughter namely XXX.

4.                 That due to inadvertence, I did not register data of birth of my daughter XXX with the concerned department at the enrolment.

5.                 Hence, this sworn affidavit is in lieu of date of birth certificate.
